The rule

Players bat in the order recorded on the official line-up card; the order carries over between innings. The batter must enter the box within 10 seconds of the umpire’s play signal. Before the pitch, both feet must be completely inside the box; touching the marked lines is allowed.

In practice

Between pitches, the batter generally keeps a foot inside, with exceptions such as a swing, a hit, a wild pitch or time being called. Hitting the ball with a foot completely outside the box on the ground, or touching home plate, can produce an out. Batting out of order is handled through the appeal procedure, not by casually changing the line-up.
